This is a very simple fluency survey that I made for school-age children and older in an effort to get a understanding of how my clients/students perceive their own fluency. This simple survey asks students to identify the types of dyslfuencies they exhibit as well as times that the student is and is not fluent. This is a simple tool that can be used for educating students/clients on what is actually happening as compared to their perception (some children are spot on with their understanding of their fluency skills while others highly exaggerate their dysfluencies). It can also be used as a way to open the very uncomfortable discussion about dysfluencies. This was created as a tool to aid in treatment.
