This informal checklist can be used during initial assessment and reassessment periods for clients who are suspected or exhibit voice disorders. The checklist includes an exhaustive list vocal abuse behaviors, which when completed provide a holistic view of a client’s vocal use. In addition to abusive behaviors this checklist also includes a number of medical conditions than can also affect vocal quality, pitch and intensity (e.g. asthma, allergies, etc.) You can rate each behavior and condition on a severity scale from 1-5 (1 being “never exhibits behavior”, 5 being “exhibits behavior always”). This checklist can be used in isolation or in conjunction with other evaluation measures as deemed appropriate.
